  1. The Great Quizoo says:

Nothing. It is merely a notification that for one reason or another, they are no longer associated. Call it leaving if you will, but it’s the same concept.

The doctor is most likely still practicing medicine, just not at that particular hospital and/or teaching institution. He may have a private clinic and still seeing patients there. Or maybe he was a specialist that visited that hospital/institution one or two days a week, and spent the remainder of the week at a different, and now he will be at the other one all the time.
There could be any number of reasons. But one thing they all will have in common is that he will no longer be seeing patients or be associated with that hospital/institution any longer.

  3. Neil says:

It means that the doctor is no longer affiliated with the hospital.
By the way, the doctor may very well never have been employed by the hospital.

A doctor is often affiliated with one or more hospitals. They are not employees of the hospital but can be considered a outside contractor who has the right and privilege to practice at the hospital.
This is usually mutually beneficial, the doctor has access to the hospitals resources and the doctor sends his patients to that hospital when necessary. Often the doctor does his billing etc through the hospital.
A public notice is a legal notice that the two are no longer affiliated.
